What To Expect at your Consultation

Being well-prepared for your initial consultation will ensure that the doctor has all of the needed information to provide the best possible care to you. It will also help alleviate any unnecessary anxiety you may be feeling prior to your first appointment. Before your appointment, verify with your referring doctor that all pertinent records have been sent, prior to your consultation.  

A procedure will not be performed at the initial visit even though you may have been evaluated by another physician. During your consultation with the physician, together you will go over health history, family history, previous procedures and any symptoms you may be experiencing. During your discussion we encourage you to ask questions. Following your discussion your physician will discuss plan of action, which may include a procedure, further diagnostic tests, follow up visits or lab work.

New & Established Patients

We ask that you arrive 30 minutes early and bring: 

  • Photo Identification
  • Insurance card(s)
  • Co-pay or payment 
  • Current list of all prescribed and over-the-counter medicines or supplements you take.

To expedite your registration please print and fill out the patient registration forms or you can fill out your patient registration form through the patient portal. If you did not receive a portal invitation to your email, please click [here] to send us a message and our office will send a link to the email provided. 

If you need to cancel or reschedule your appointment, we ask that you kindly give us 24 hours notice. Repeated last-minute cancellations (less than 24 hours notice) or missed appointments, may result in dismissal from the practice. If you are late for your clinic appointment, you may be asked to reschedule.

Telehealth Visits

Telehealth visits are available for new and existing patients. This convenient option allows patients to have a one-on-one visit with their specialty provider, while in the comfort of their own home. Conditions and symptoms that require a physical exam will need to have an in office visit. 

Getting ready for your telehealth visit

  • Ensure you have a strong WIFI connection
  • You will need either a computer, tablet or smart phone that both audio & video capability.
  • Find a quiet place free from distractions. If there are numerous interruptions, your provider may cancel your telehealth appointment and request that you have an in person visit.
  • Log into your patient portal to update your medical history prior to your appointment. Check your email for your portal invitation or call our office to receive an invite to our portal system. 
  • Prior to your appointment you will receive an email with instructions on how to login.


Secure Patient Portal 

We are happy to introduce you to a solution that enables you to communicate with our practice quickly and securely using the internet. Through our patient portal you are able to update personal and medical information, view records, confirm appointments and communicate with our staff and your provider.
